MWRD requests the below information to be completed before new service can be started. All service requests are normally completed Monday through Friday (excluding holidays).
All requests made after noon (12:00) will be processed the following business day. If your service request needs to be completed between noon and 3:00 pm please contact customer service at 615-848-3209.
In addition to submitting this form please email a valid photo ID (i.e. driver’s license, state issued ID, military ID, passport) to mwrdcontact@murfreesborotn.gov, or fax to 615-907-2254. Documents will need to be received before request can be completed.
MWRD does not require a deposit to start service. However, there is a $40 non-refundable service fee for all new or transferred service requests. The $40 fee will be added to the first bill. Liability for service for this address will continue until notification is received to terminate the services. You must contact Customer Service @ 615-848-3209 to terminate service and provide a disconnect date and forwarding address to send the final bill.
A copy of a valid photo ID or driver's license MUST be emailed to mwrdcontact@murfreesborotn.gov after completion of this form!
